Finding the most appropriate mental health clinician can be very difficult and confusing. Our staff will do everything possible to connect you with the most appropriate clinician for your needs. To make this process more efficient we ask that you complete a brief “intake” interview.

You can then complete the form and click on the submit button at the bottom of the form. You may also print the form and submit by facsimile or by mail. Instructions are also located at the bottom of the intake for your convenience.
PLEASE NOTE: These PDF forms are setup to be opened, completed, and submitted online in a safe and secure manner. Prescription Refill Requests (existing patients only)
If you are a current patient and would like to request a prescription refill please download and complete a Prescription Refill Request Form to submit online. Once you complete the form, click the submit button at the bottom of the form. You may also fax, or mail the form in or call the main office at 781-551-0999 ext 405.
Parent Apart Program. You can download and complete a Registration Form to submit online. Once you complete the form, click the submit button at the bottom of the form. You may also call, fax, or mail the form in. Check for upcoming dates and information.
Once we have received your request it will be reviewed by our staff and we will attempt to match you to an appropriate clinician. One of our staff will contact you by telephone or email, usually within 5-7 business days and inform you as to whether we can meet your needs.
