Finding the most appropriate mental health clinician can be very difficult and confusing. Our staff will do everything possible to connect you with the most appropriate clinician for your needs. To make this process more efficient our staff will complete a brief "intake" interview. You may do this on the telephone or may print out the intake form and mail or fax it in.

Once you have completed the intake form it will be reviewed by the clinician you requested or one of our doctors who will attempt to match you to an appropriate clinician. One of our staff will call back, usually within 4-5 business days and inform you about our ability to meet your needs.

Please call 781-551-0999 ext. 201 between 9:30 a.m. and 5:00 p.m. Monday through Friday and they will take the intake information. Staff may also be available on Saturdays between 10:00 a.m. and 1:00 p.m. Please review the online intake form and have the necessary information ready when you call.
